Applying a systems lens to health care delivery has enhanced the medical profession’s recognition of health disparities and the tremendous impact of upstream drivers and social determinants of health. An increased burden of chronic disease and the demand for value-based care are also driving a focus on population-level interventions in addition to the delivery of care to individual patients.
